To apply for a birth or death certificate, fill out the forms available on the Miami County Public Health webpage at https://www.miamicountyhealth.net/birth--death-certificates. These forms can be mailed or faxed in with payment. Phone orders can also be taken by calling MCPH offices Monday through Friday 8:00 a.m. to 3:30 p.m. by calling 937-573-3500.

If you have a scheduled WIC appointment, please call (937) 573-3549 so an employee can give you access to the building. Only the woman participant or parent/guardian of a child should come in the facility for the appointment. If you don’t have an appointment and are in need of WIC services, please call the phone number listed above.
If you have an appointment with Nursing, please call 937-573-3518 so an employee can give you access to the building. Only the person with the appointment may enter the building.

The Miami County Public Health Board of Health has temporarily postponed the public hearing for Environmental Health fee changes in the Private Water System Program until further notice.
