There are four ways in which to obtain a birth or death record in Ohio. Choose the method that best suits your needs:

Local Vital Statistics Office - You can obtain birth and death records from Miami County Public Health's offices regardless of where you were born in Ohio, provided there is no curent court or legal action.

  1. Walk-in: Same day service is available to walk-in customers. This is the fastest way to obtain a birth certificate. When you arrive, you will complete an application and pay the required fee. Regular Office hours: Monday - Friday: 8:00 AM- 3:50 PM (Note: We are closed all Federal Holidays)

    We accept cash, check, Visa or MasterCard (Debit or credit) or money orders (in U.S. funds)

    Approximately wait time is 10-15 minutes.

  2. FAX: We will accept faxed request for birth records with credit or debit card payment. Download the application from our site and complete all information. ALL APPLICATIONS MUST CONTAIN A SIGNATURE.

    No request will be filled if the application is not complete. Use the VS application tab on the menu to download an application.

  3. Mail: - Turn around: SAME DAY. The day we receive the request (Use the VS application tab on the menu to download an application.) the document is processed and sent out in that evenings mail. The type of return mail chosen by the applicant determines delivery. Next day is available at an extra charge. (You are responsible to check with your local post office to verify that your zip code and ours (45373) is within the NEXT DAY guaranteed region. We also have Priority mail, which usually takes 2-3 days. Or enclose a self-addressed stamped envelope for us to return your request to you. You are urged to allow sufficient time for delivery for all birth/death records. Mail a check or money order for each certificate plus additional charge for preferred return postage. Postage charge is per envelope …NOT for each certificate requested.
  4. At the State Department of Vital Statistics

    1. Walk-in - Same day service is available to walk-in customers. When you arrive, you will complete an application and pay the required fee.

    3. USCerts or VitalChek Network - Obtain a certified copy of a birth or death certificate via credit card using either of these two vendors. Additional service charges will be applied to each order. Federal express delivery can also be selected. Turn around is estimated at 2 to 3 weeks from the day the request is received, not including additional time for mailing.
